Registration Payment Information |
| We accept payment for registrations in the form of check, MasterCard, VISA, and Purchase Orders. |
| Early Bird registration must be postmarked on or before September 30, 2010. |
| Please submit an individual registration form for each registrant. |
| Full payment must accompany all registration forms. Registrations will not be processed without full payment included. (Exceptions for Purchase Order payments only.) The total dollar amount of Purchase Order must be received must be received no later than 60 days from the date of the invoice. |
| Conference refund requests must be in writing and postmarked by October 10, 2010. A $15.00 processing fee will be assessed. Meal fees are nonrefundable. |
